Authorization and Consent On your child's enrollment form, you provided the names of two adults who have permission to transport your child should you be unable to do so. Your enrollment form also listed a "code word", which must be known by anyone other than a parent who is transporting your child. Should this information change, please notify the Director immediately. If you are unable to pick up your child, you must provide prior written notification as to who will be picking up your child. A photo ID, in addition to the code word, will be requested from that person. If circumstances do not allow you to send in a note, you may call the Director at (508) 533-5484. Should you decide to carpool with another parent, please notify the Director in writing. If there is a person who should not be allowed to transport your child under any circumstances, please notify the director.
If possible, please notify the Director if your child will be absent. Should your child become ill due to a communicable disease (e.g., chicken pox, strep throat, conjunctivitis), please notify the Director as soon as possible so that we may alert the families of the children who may have been exposed.
Parents will be asked to provide an approved snack for their child's class approximately three times during the year. A calendar designating your "snack day" will be provided at the beginning of the year, along with a list of appropriate snacks. We will provide snacks during the month of September. Please remember that our facility is peanut-free. Parents may be asked
occasionally to donate paper towels, tissues, baby wipes, etc. Emergency Procedures In the event that a child becomes injured at school, the following procedures will be followed: Minor Injury: Appropriate first aid will be administered to minor cuts, scrapes, and bumps. Parents will be notified at pick-up time. Major Injury: Appropriate first aid will be administered. Emergency personnel will be summoned, and parents will be notified. The Director will accompany the child in the ambulance.
