Administration of medication: requirements

PRESCRIPTION MEDICATION

written parental authorization
written order of a physician (the label on the medication is acceptable)

NON-PRESCRIPTION MEDICATION

a signed statement by the physician, listing the name, dosage and criteria for administration of the medication (valid for 1 year from the date it was signed)
a signed statement from the parent authorizing administration in accordance with the written order of the physician (valid for 1 year from the date it was signed)
The parent will be notified before the medication is administered

TOPICAL NON-PRESCRIPTION MEDICATION

a signed statement form the parent listing the specific topical non-prescription medications to be administered and the criteria for administration (valid for 1 year from the date it was signed)
must be administered according to the directions on the original container unless authorized to do otherwise by a written order of the physician

ALL MEDICATIONS

Expiration dates will be checked and parents notified about expired medications.  Expired medications will not be given.

STORAGE OF MEDICATION

all medication must be kept in its original container labeled with the child's name, the name of the drug, and the directions for its administration and storage (except for topical non-prescription medication not applied to open wounds, rashes or broken skin)
all medication is to be kept in the locked drawer of the desk in the nursery school office
unused medication will be properly disposed of (down sink or toilet) or returned to the parent (not via school bag)

DOCUMENTATION

All medication administered will be documented on the child's record.  The following information well be included:  The name of the child, the name of the drug, the date, time and dosage, and the signature of the staff member giving it.  Parents will be notified in writing each time a non-prescription medication such as sunscreen, bug spray and other ointments which are not applied to open wounds, rashes or broken skin do not need to be documented.
